Kybella® is an injectable treatment for fat beneath the chin, commonly called submental fat or a double chin. Its active ingredient is synthetic deoxycholic acid, which destroys treated fat cells so the body can gradually clear them.
But fullness beneath the chin is not always just a fat problem. The appearance of the neck and jawline can also be influenced by skin laxity, muscle anatomy, chin projection and the way the tissues of the lower face and neck come together.
That is why our aesthetic providers begin with the anatomy rather than assuming every double chin should be treated with Kybella.

The amount of fat matters, but so do skin quality, chin projection and the underlying anatomy of the neck.

When the skin and underlying neck anatomy are appropriate, Kybella can gradually reduce this localized fat without surgery. Patients with a larger amount of submental fat or those looking for a more immediate change may be better candidates for submental liposuction.
Sometimes the fullness beneath the chin is not primarily fat. As skin loses elasticity, it can become loose along the jawline and neck, creating heaviness or obscuring definition even when relatively little excess fat is present.
Kybella reduces fat, but it does not remove significantly loose skin. Depending on the degree of laxity, treatment may involve non-surgical skin treatments for more subtle changes or a neck lift when loose skin and deeper signs of neck aging are the primary concern.
The shape of the chin, jaw and deeper structures of the neck has a major influence on the profile. A smaller or recessed chin can make the area beneath it appear fuller because there is less projection separating the chin from the neck. Deeper neck anatomy can also contribute to fullness that will not be corrected simply by reducing superficial fat.
Depending on what is creating the concern, treatment may involve chin filler or chin augmentation to improve projection, while more significant anatomical changes of the neck may require surgical treatment. In some patients, reducing submental fat is only one part of improving the overall profile.
Identifying that difference matters. The goal is not simply to reduce fat. It is to determine what is affecting your profile and choose the treatment that can actually improve it.
Kybella® is injected directly into the subcutaneous fat beneath the chin. Deoxycholic acid disrupts the membranes of the treated fat cells, causing those cells to be destroyed and gradually cleared by the body.
Treatment is planned across the area rather than placing a single injection into one spot. Our aesthetic providers evaluate the distribution of fat and the surrounding anatomy before determining where Kybella should be placed and how much treatment is appropriate.
Once destroyed, the treated fat cells can no longer store fat.
Kybella is usually approached as a treatment series rather than assuming one appointment will produce the desired change.

The objective is not to reach a predetermined number of sessions. It is to use the amount of treatment appropriate for your anatomy.
Kybella and liposuction can both reduce fat beneath the chin, but they are very different treatments.
Kybella allows our aesthetic providers to gradually reduce a localized pocket of submental fat without surgery. Liposuction physically removes fat and can create a more immediate and extensive change when the anatomy calls for it.
Neither option is automatically better. The amount of fat, skin quality, neck anatomy and the degree of change you want all matter when deciding which approach makes sense.
Before and After
Kybella results develop gradually as the body clears the treated fat cells. As submental fullness decreases, the transition between the chin, jawline and neck can become more defined. Because every patient begins with different anatomy and a different amount of submental fat, the degree of change will vary.
Kybella can be a good option when a localized pocket of fat beneath the chin is contributing to unwanted fullness and the patient prefers a non-surgical approach.
But wanting to get rid of a double chin does not automatically make someone a Kybella candidate. If loose skin, neck anatomy or another structural issue is contributing significantly to the appearance, reducing fat alone may not create the result you are looking for.
You do not need to decide whether you need Kybella, liposuction or another treatment before your appointment. That is what the evaluation is for.
The injections themselves are relatively quick. The part patients should understand before choosing Kybella is what happens afterward.
Swelling is expected because Kybella creates an inflammatory response as the treated fat cells are destroyed. The area can also feel tender, firm, numb or bruised temporarily. For some patients, the swelling can be noticeable, particularly after the first treatment. Our aesthetic providers will explain what to expect and provide aftercare instructions based on your treatment.
A more defined chin and neck is not always about removing more fat. It starts with understanding what is creating the fullness in the first place.
For the right patient, Kybella can gradually reduce a localized pocket of submental fat and improve the transition from the chin to the neck without surgery. For someone else, skin laxity, chin projection or deeper neck anatomy may be more important to the appearance of their profile.

How does Kybella work?
Kybella contains deoxycholic acid, a naturally occurring molecule that breaks down and absorbs dietary fat. When injected into the area under the chin, it destroys fat cells, which are then naturally eliminated by your body.
How many Kybella sessions will I likely need to see a noticeable reduction?
Clinical data from the REFINE trials show that most patients see meaningful improvement in 2 to 4 treatments, even though up to 6 sessions were allowed.
Is Kybella permanent?
Yes. Once Kybella destroys the fat cells, they cannot regenerate. With a stable weight, the results are considered long-lasting or permanent.
What does Kybella feel like? Is it painful?
You may feel a brief stinging or warmth during injection. A numbing agent or ice pack is typically used beforehand to minimize discomfort. Mild swelling and tenderness are common for a few days afterward.
Are side effects like swelling and bruising normal, and when do they resolve?
Yes. Swelling, bruising, numbness, tenderness, and firmness are common early after treatment. In the REFINE trials, the majority of these side effects were mild to moderate and resolved over days to weeks.
Does Kybella only work for the area under the chin, or can it be used elsewhere?
Although Kybella is FDA-approved for submental fat (the “double chin”), off-label applications—like reducing fat in small facial or contour areas—are being explored. Early reports show similar safety and satisfaction in select cases.
How long do the results last? Will fat return under my chin?
Follow-up studies show that most patients maintain submental fat reduction for at least 3 years after their final Kybella session.
Will Kybella treatment be very painful or uncomfortable?
In trials, patients generally rated peak pain within 1 to 5 minutes as mild on visual analog scales. Using techniques like buffered lidocaine with epinephrine and preemptive cold packs can reduce pain by up to 40%.
What is the mechanism by which Kybella works to reduce fat?
Kybella contains deoxycholic acid, which causes adipocytolysis—the breakdown of fat cell membranes. The body then clears the cell debris naturally, and tissue remodels over time.
Are there risks of nerve weakness or asymmetric smile after Kybella?
Yes, in clinical trials there were isolated cases of temporary marginal mandibular nerve weakness, which manifested as an asymmetric smile. Most resolved on their own within days to months (median ~44 days).
How should I prepare before a Kybella appointment to reduce side effects?
Common recommendations include stopping anticoagulant or antiplatelet supplements ~7–10 days before injection, applying cold packs, and using buffered local anesthesia. These measures help reduce bruising, swelling, and discomfort.
